• Job Title: Locum Tenens Nurse Practitioner – In-Home Health Risk Assessments
• Location: Elkhart County, Indiana (zip 46516) – Indiana healthcare jobs
• Position Type: PRN, part-time, contract (26 weeks)
• Coverage Area: County-based; travel up to 120 miles/day required
• Shifts per Week: 5 (flexible scheduling)
• Shift Duration: 8-hour field days; start at 7:00 or 8:00 AM
• Minimum Availability: 4 full days/month (weekday coverage preferred; weekends a plus)
• Estimated Weekly Pay: $500 - $600
• Travel Stipend: $60/day for mileage and gas

Nurse Practitioner Qualifications

• Active, unrestricted Nurse Practitioner license in Indiana (ANCC, AANP, or AACN Board Certification required)
• At least 1 year of NP clinical experience (new graduates not eligible)
• Accepted specialties: AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner)
• Experience with geriatric patient care and in-home clinical assessments preferred
• Ability to travel independently to patient homes, including rural and underserved areas
• Strong adaptability, reliability, and commitment to patient-centered care
• Professional attire or dark-colored scrubs (no lab coat required); must provide own basic medical tools

Healthcare Job Responsibilities

• Perform annual in-home Health Risk Assessments for Medicare/Medicaid members (1:1 visits, no follow-up care)
• Conduct physical exams, medication reviews, and assess social/environmental factors impacting patient health
• Complete basic labs (A1C, fit tests – no blood draws required)
• Document and submit assessments using a company-issued tablet (same-day submission required)
• Travel daily within assigned Elkhart County territory for patient visits
• Adapt to changing schedules, route challenges, and occasional no-shows or cancellations
• Maintain high-quality clinical documentation and adhere to all healthcare standards
• Complete onboarding and training modules (~20 hours within 14 days)
• Collaborate with Operations and Clinical Services teams for support and quality assurance
